Having gathered adequate, relevant and specific information on the incident is crucial for our investigation.
Please provide the following details regarding your concern:
- Date/Time of the situation
- Names of the associates involved including those who may have knowledge or witnessed the situation you are describing
- The location where the situation occurred
- Have you reported your concern to anyone in the company? If so, to whom and when?
